It would be a piss poor company that doesn't take disciplinary action against the driver. P45 seems justified.
And it would be a good example if said company also posted a comment here (with duplicate on their website to confirm originality) that they agree that the driver failed in their responsibility to the general public (including road users) and to the company.

Seriously.. what the fuck was the skip lorry driver playing at? I'd say ride primary from the outset but it's no guarantee you won't get idiots like this, trying to fit into the same lane. That's just insane driving on their part and if I were the boss of the company involved I'd have grave reservations (literally), regarding their driving ability.. that is no way to pass a cyclist. Lucky you weren't wiped out. Take care my friend.
