Driver should be punished for failing to stop at the scene of an accident no doubt.

The driver had no way of seeing the cyclist coming so should have proceeded with caution. Likewise the cyclist if he/she was reading the road ahead could see the road layout and the gap in the traffic so should also have proceeded with caution.
Shall we blame the white van driver for blocking both their views? :)
Road design like this also makes it very difficult for drivers to see you, even if they slow down to a crawl speed. Now just imagine going at say 20 mph and they're slowly turning.. it makes no difference; you will most likely collide. Bloody stupid road design in this country..
Hope you are alright. This is why one should NEVER put their faith in cycle lanes.. many of them are badly designed and give a false sense of security (not just to cyclists but also drivers). This also shows the dangers of side roads.. always, always, slow down to a point where you can stop if need be especially if like here your view is obscured. A case of bad road design meets too much vehicular traffic. The outcome is inevitable..
